I have intentionally not included this e-mail form on my web page for two reasons. There is already a far superior comments facility there, and secondly, I make a point of never posting my e-mail address in web pages as I already have enough trouble dealing with spam.
But, as proof that I can hack together a HTML comment form, here it is:
